Registration Forms & Requirements - for Kindergarten students only!

Registration at Loma Prieta Joint Union Elementary School District (LPJUSD) is a two-step process.  The first step must be completed online.  Please submit your online registration before completing the second step, which is submitting the hard copy packet.  After submitting the online portion of the registration process, parents must complete and submit the hard copy packet (documents listed/linked below) to complete the packet, which is the final step in the registration process. The registration of your child at LPJUSD is not complete until both online and hard copy registration forms are validated.

Registration for the 2024-2025 school year opens on March 1, 2024. (If you are interested in registering your student after February 1, 2025 you will need to call the registrar)

After completing the online registration forms, parents will need to come back to this site to download the supplemental forms below.  

Parents may download and print the following forms or pick up a packet at the district office.

Hard Copy Packet for 2024-2025:

• Photocopy of your child’s Original Birth Certificate or Passport

  • Students born on or before 9/1/2019 are eligible to attend Kindergarten.

• Photocopy of your child’s Immunization Record, as of the registration date.

  • An updated copy will be required later

• Photocopy of any Shared Custody arrangement for your child(ren)

  • Legal Custody Agreement at the time of registration is required
